New Patient Information

On your first visit to the Stein Medical Institute, we ask that you try and arrive approximately 15 minutes prior to your scheduled appointment to allow enough time to complete our New Patient Information Packet.

Just in case you are one of those people that tends to run late or gets crunched for time, we have provided you with access to our New Patient Information Packet online in PDF format.

Simply print out the forms, complete them on your own and bring the completed forms to your first appointment.

Please note the following:

• You will need Adobe Acrobat Reader to view the New Patient Packet. If you do not have Adobe Acrobat, download a free copy now.
• All forms need to be completed. Please do your best to complete every form and question within each form. If you have questions, feel free to contact us.
• Even if you complete the forms prior to arriving to your first appointment, we recommend arriving at least 5 minutes early, so that our staff can ensure you have filled out the forms completely and accurately.

New Patient Packet

• Personal Medical History: Provides us with a complete background of your medical history 
• Client Confidentiality: Additional medical history pertaining to allergies 
• Patient Photo Release: Provides us with your level of consent to use your photos on our website or within other media for our practice
• Audio Visual Consent: Provides us with your level of consent to use your photos on our website or within other media for our practice 
• HIPPA: A required medical form allowing us to request information about your medical history from your other doctors when needed in order to provide you with the best care possible.  
• Fee Policy: Outlines our Standard Policy on fees for our services 
• Medical Records: A form we may send to your other doctors or previous doctors to request information about your medical records
